❓What is Form 6?
All candidates running for office in Florida are now required to complete Form 6 - Full and Public Disclosure of Financial Interests. This form provides a snapshot of your finances from the previous year. A verification receipt from the Commission of Ethics must be turned in to the Division of Elections, Supervisor of Elections, or City Clerk with all other qualifying documents.

What am I disclosing?
1️⃣ Your net worth
- Sum of all assets, subtract all liabilities - not just what is individually reported or disclosed.
2️⃣ Assets
- Intangible, tangible, and real property
- Value of all household goods and personal effects
- Art, vehicles, jewelry, clothing, coin collections, house equipments
- Law requires a good-faith estimate
- These are not itemized if they aren’t for investment
- $1,000 and over individually are reported
- Most common: bank accounts, real property (house/land), stocks
- Description of assets and values
3️⃣ Liabilities
- Excludes: credit cards, life insurance policy, taxes owed, contingent liabilities
- Most common: mortgages, car payments, student loans
4️⃣ Primary Sources of Income
- Income over $1,000 - anything that would constitute gross income on federal tax returns
- Disclose Public Salary - gross income
- Does not describe how you made the money
